This form is to help Kiley and Bernie figure out what relays you want to swim and your times. There are printouts available at the pool too. Please return your completed forms to the black box at the WeHo pool no later than Friday June 13, or you can eMail your response to Bernie at BernieJL@aol.com. Bernie can also answer any questions you may have.
